Contact Us Risk Management Department Staff The Risk Management Department is comprised of staff who collectively deliver a broad spectrum of risk management expertise. Please consult the Risk Management Staff Subject Matter Matrix to determine which member(s) of our team to consult for your risk management needs.
To File an Insurance Claim To begin an insurance claim process, complete the Property Damage/Loss Claim report (available in the Finance Division Forms Library). This Word document (.doc) should be emailed as an attachment as per the instructions on the form. Supporting documentation can be emailed or faxed to Risk Management. Most claims should include digital pictures, a campus safety and/or police report, and copies of paid invoices as appropriate. NOTE: One person from the department must be designated as the departmental contact. This person will be responsible for providing information to Risk Management when requested, conducting any investigation that is necessary, and acting as the point person throughout the claims process.
