To File an Insurance Claim
The University loss sharing deductible on all Property Damage/Loss claims is $2,500. Claims with a total loss under this amount will not eligible for reimbursement from Risk Management.

To begin an insurance claim process, complete the Property Damage/Loss Claim report (available in the Finance Division Forms Library). This Word document (.doc) should be emailed as an attachment as per the instructions on the form. Supporting documentation can be emailed or faxed to Risk Management. Most claims should include digital pictures, a campus safety and/or police report, and copies of paid invoices as appropriate.

NOTE: One person from the department must be designated as the departmental contact. This person will be responsible for providing information to Risk Management when requested, conducting any investigation that is necessary, and acting as the point person throughout the claims process.
