University ID Cards To obtain a card, whether your first or a replacement card, your employment status must be authenticated online. If you require a replacement card due to loss or theft, please speak with Lauren Santiago to make sure that this authentication process is done prior to visiting the ID Center. A signed letter on departmental stationery is no longer required. Please note that when you go to the ID Center to get a new or replacement card, you must have with you a means of verifying your identity. 330 5th Avenue ID Cards Building ID cards must be carried at all times. If you damage or lose your building ID card, please see Idelsi Botex regarding the issuance of a new card. Employees may not request a new ID directly from the building office – the request must first go through the above named individual. Termination If you are no longer going to be working in 330 5th Avenue, but are going to another position elsewhere in the University, you must turn in your 330 5th Avenue ID card to Idelsi Botex, Lauren Santiago, or Kate Sheeran on your last day in this office.
